Before the first report, initialized is false and schedule and latestAudit still describe the current review, including failed or waiting work. A requested revision resolves directly from organization-scoped immutable storage even after leaving the recent-history list. Missing revisions return 404; integrity failures return 409. Version-2 history is normalized on read without rewriting stored bytes.
